1. Introduction - To apply for a scholarship from Jewish Family and Children's Service:

Carefully review all guidelines for eligibility as outlined on the next page. If you meet eligibility criteria, you may then complete all applicable questions on the application. Please be sure to carefully read all instructions and answer questions accordingly.

Once you begin this application, you must complete it and send. There is no option to save your work and go back at a later time. We recommend that you print each page of the survey and review it carefully before beginning to fill in the application.

You must also mail us the following supplemental documents no later than April 1, 2012:

* A copy of the Student Aid Report (SAR) containing the Expected Family Contribution (EFC) from the the 2012-2013 Free Application for Federal Student Aid (FAFSA).

* If received, a copy of your Letter of Acceptance from the college you will be attending.

* A copy of your high school transcript for every academic year completed.

* One letter of recommendation from your school principal, a teacher, or advisor.
