Please use this form to help us update your contact information and to let us know if you will be attending and if you will need a hotel sleeping room for Thursday night, November 3rd. Sleeping rooms will be available only for people who have to travel more than 50 miles. We will be paying for the hotel rooms through a purchase order so you need to mark on this survey if you will be attending and if you will need a hotel room. Reimbursement for additional travel expenses will be available through the University of South Florida, so please mark on this survey if you need general travel reimbursement (meals, mileage, tolls, etc.). Remember, travel reimbursement is only available if you are traveling more than 50 miles one way.
