Automated External Defibrillator (AED) Rebate Application

The District of Columbia Fire and Emergency Medical Services Department by authority of DC Official Code § 7-237104a, has established an Automated External Defibrillator (AED) Incentive Program to encourage purchase and installation of AEDs in publicly accessible locations on properties owned or leased by small businesses, places of worship, small multi-unit apartment, condominium, and cooperative buildings, and non-profit organizations.

Qualified organizations may receive up to a $400 rebate per AED (or up to $750 per building address with multiple AEDs), if such equipment is registered with DCFEMS and made available for public use.

Once you have registered your AED by completing the DC Fire and EMS Department Public Access AED Data Request Form, please complete each section of this form.

What you will need to successfully complete this application:
1) AED Coordinator's and building information
2) Proof of building ownership OR permission from the owner to install an AED
3) Copy of building's Cardiac Emergency Response Plan (CERP)
4) Photo of properly installed AED
5) Copy of annual maintenance plan or commitment to obtain maintenance plan
6) Tax ID Number or District supplier number
