Hip Fracture Rehabilitative Care Resources Submission
Thank you for assisting with the development of a resource of hip fracture tools and resources!
During this time of system change, including the provision of care through a pandemic and a transforming health care system, a new way of organizing and delivering care is in development. New and innovative delivery mechanisms are being implemented, including virtual care. In an effort to support the ease of access and sharing of rehabilitative care best practice resources for the frail senior hip fracture population, the RCA Hip Fracture Best Practices Task Group is developing a centralized web-based resource of tools and resources that support best practice care for hip fractures. RCA is currently seeking resources or tools across locations and processes of care to populate the resource which will be shared on the Rehabilitative Care Alliance website.
We are therefore requesting your assistance. If your organization has developed and/or implemented resources or tools that are evidence-based and align with best practices, such as patient & family education materials, care pathways and assessment tools, we would appreciate if you could please do the following:
Complete the following survey and upload each tool/resource below.
You may submit up to 5 tools/resources in this survey. To submit more than 5 tools/resources, please open a new survey to complete your submission.
If you have any further questions or comments, please email Gabrielle Sadler, Project Manager gabrielle.sadler@uhn.ca
