The Community Star Recognition Program, hosted by the National Organization of State Offices of Rural Health (NOSORH), honors individuals and organizations that are making a lasting difference in rural communities. Selected stories will be featured nationally on National Rural Health Day this November.

This brief nomination form will help us learn more about your nominee and why they deserve to be recognized as a 2025 Community Star.

🟢 Nominees must meet the following criteria:
• Be located, working, or volunteering in a rural area
• Be an individual, organization, or collaborative group making a positive impact on rural health
• Be involved in rural health services, outreach, education, partnerships, or other community efforts
