You are under no obligation to answer the questions presented in this survey and your participation is voluntary.  If you are not comfortable answering any question, you may simply decline to answer.  There are no alternative means available to participate and no compensation is being offered to participants.  No identifying information is being gathered about you during this survey; therefore your responses are confidential.

Criteria for participation:

You are/were circumcised 
You have experienced distress related to being circumcised
You have NOT attended therapy to address this concern
You are a resident of the United States
