This survey is intended to be completed by individuals living in long-term care facilities. If you are the family member of someone living in a long-term care facility or are a long-term care Ombudsman program representative, you are encouraged to assist residents in completing the survey. However, please ensure that responses are the resident's own. In the survey, we use the term "facility" to reflect long-term care settings including nursing homes and assisted living facilities.
