The Best Wellness Employer Survey offers a valuable opportunity to showcase the achievements of your worksite wellness program and its positive impact on employee health. With rigorous evaluation standards across assessment, strategy, implementation, and evaluation, this survey enhances the credibility of your wellness initiatives and validates your commitment to employee wellbeing for your team, senior leadership, and external stakeholders.

Your participation also contributes to the advancement of worksite wellness by providing essential data and insights on current practices and wellness outcomes. All responses from applicants are strictly confidential and anonymous.

This survey will include 5 sections:

- Organization Information (not scored)

- Assessment (scored)

- Strategy (scored)

- Implementation (scored)

- Evaluation (scored)

Survey completion will take up to approximately 25 minutes. Please plan to complete the survey in one sitting to ensure your responses are saved.

While completing the survey, it may be helpful to have the following available, if possible, for reference:

- Health Risk Assessment Data

- Wellness Program Budget

- Wellness Program Evaluation Data (e.g., year end data, KPIs, ROI, etc.)
