Parent/Professional Advocacy League (PPAL). a family organization, is conducting a survey of families who have children or youth with special needs, especially behavioral health needs. The purposes of this survey are 1) to better understand the struggles families face in coordinating the many parts of their child or youth's care and 2) to ensure that this information is reported in order to impact both policy and practice.
Health care reform is happening NOW across the country. Medicaid and private insurance companies are busy testing different ways to deliver care to children and adults alike. In many of these models, children, youth and parents are supposed to have a central role. These models of care (often called medical homes or health homes) are still under construction so it's important to hear from you.
The survey should take 12 to 15 minutes to complete. All responses to this survey are ANONYMOUS and results will only be reported in aggregate form, such as percentages.

At the end of the survey is a link to a raffle. There will be a weekly drawing for $25 gift card.
