Greetings!

This survey is your proposal to participate in the next phase of the State Alliance of Michigan YMCAs’ (State Alliance) Michigan Health Endowment Fund (MHEF) project. The information you provide in this survey will be delivered to and used by the State Alliance to allocate funding awards for Phase 3, which spans May 1, 2016-December 31, 2016. In addition, the data you provide will be used to create a work plan for your YMCA. Throughout the grant period, Altarum Institute will conduct progress surveys to assess the extent to which you are completing the activities outlined in your work plan.

The survey is organized into four sections—HOST Summer Programs, HOST School Year Programs, Michigan Swims, and Healthy Living Capacity Building. You will only need to complete the sections that you are applying for funding to conduct activities in during Phase 3 May 1-December 31, 2016. Within each section, you will be asked to provide detailed information regarding what you intend to accomplish and where.

After survey results are reviewed, the State Alliance will notify you of your funding amount. You will also receive a copy of your work plan after funding awards have been announced by the State Alliance. If at any point during Phase 3 your work plan changes, such as due to a negotiation with the State Alliance, please notify Altarum (Kara.Wise@altarum.org) immediately so that we can update your work plan and ensure that your Y is not being held responsible for activities that you no longer intend to accomplish and so that we can accurately represent this project to the State Alliance’s funders and partners.
