Operation I.D. is an initiative sponsored by the Department for Community Based Services aimed at increasing access to State Identification Cards for youth in state's care. As part of the initiative, DCBS has partnered with the Kentucky Transportation Cabinet to provide foster youth with a free electronic option to request a state I.D. (New State I.D.s only. To renew or replace State I.Ds, youth will still need to visit a KYTC Driver Licensing Regional Office).
Eligibility for this service includes current foster youth between the ages of 16 and 20 years old.
Instructions: Carefully answer each question, ensuring the information entered is correct. The request must include a legible copy of the youth's social security card and birth certificate. An eligible picture to be used for the ID must also be included. The Identification Card will be mailed to the address included in the request within 4 - 6 weeks.
