Survey Purpose and Confidentiality:

Purpose: 
The purpose of this survey is to hear from local employers about their experiences related to Substance Use Disorder* and the Workplace.  

Information from this survey will be used to develop training and workplace policies that support people with substance use disorder, and provide a safe and healthy work environment for all staff.

*Substance use disorder, or addiction, is defined by the National Institute on Drug Abuse (NIDA) as a “chronic, relapsing brain disease that is characterized by compulsive drug seeking and use, despite harmful consequences.” Substance use disorder is a brain disease because drugs can cause long-lasting changes to the way the brain functions.

Confidentiality: 
Your participation in this survey is 100% confidential. Your employer will not know if you participate, and they will not see individual answers. If you choose to provide information that could be linked to your identity or your place of work, the information will be removed from the overall survey results to ensure confidentiality.
