2023 Community Needs Survey

Part 1: Background Information

The Area 12 Agency on Aging (A12AA), the Blue Zones Project, and the A12AA Advisory Council, are conducting a Community Needs Survey to identify the needs of older adults in Alpine, Amador, Calaveras, Mariposa, and Tuolumne Counties.

The target audience for this survey is age 50+, adult caregiver age 18+ caring for person age 60+, relative age 55+ caring for grandchildren and a resident of Alpine, Amador, Calaveras, Mariposa or Tuolumne Counties. Submit survey by October 10, 2023. The survey is annonymous.
1. What organization would you consult when in need of resources for yourself, a family member or neighbor? For example: transportation, home delivered meals, home repair.
