Informed Consent

Tania Rosa, a post-professional Doctorate of Occupational Therapy candidate with Quinnipiac University, would like to invite you to participate in a research study to examine the role of occupational therapy professionals in trauma-informed care within school settings.  Please read this following consent form carefully before agreeing to participate in this study.

Background Information: The purpose of this research is to gather data on the perception, knowledgebase, understanding, and role of OTPs in the use of trauma-informed care and practices within school-settings.

Procedures: An online survey will be completed using the survey site SurveyMonkey.  If you agree to be part of this study you be asked to follow the link below to be taken to the survey.  The survey consists of 25 questions and will take approximately 15 minutes to complete.  It will include basic demographic information, as well as, close-ended questions related to your current roles and practice within your school setting.  Please feel free to share the survey link with fellow occupational therapists or occupational therapy assistants that work within school-based practice to help obtain a thorough understanding of current practice in the occupational therapy field.

Voluntary Nature of Study: Participation in this study is completely voluntary in nature.  It is solely your decision and your choice to participate in the survey.  Your decision will not affect you in any professional or personal manner.  If your initial decision is to participate, you may still withdraw from the study at any time, and you can skip any question that you do not wish to answer.

Risks and Benefits of Being in the Study: There are no profound or serious physical, emotional, legal, financial, or social risks/burdens associated with participation in this study.  If, however you feel any emotional stress or anxiety during the survey you may stop participating in the study at any time.  There are no specific benefits to you for participating in the study. 

Compensation: You will not be provided with any form of compensation if you participate in this study.

Confidentiality: Your responses will be kept confidential by the investigators.  All data will be stored in a password protected electronic file.  At the end of the survey you will be asked if you are interested in participating in an additional follow-up interview that will then be transcribed.  If you choose to provide contact information, your survey responses may no longer be anonymous to the researcher.  However, no names or identifying information would be included in data collection, and your survey and interview responses will remain confidential. Access to the information will only be available to primary investigator and co-investigator.  The results of this study are for education purposes only and all data will be reported in total.

I have read the above information.  I consent to be a participant in this study.  I understand that I can withdraw from participation in this study without any consequences at any point.
