Informed Consent for Embudo Valley Library Survey

Purpose. The Embudo Valley Library Board is considering making changes to the appearance and functioning of the area behind the library.   Embudo Valley Library is conducting this research with the help of Felicity Fonseca, who is a student in the UNM School of Public Administration.    
Why you are asked to participate. People are asked to participate who are residents of Dixon, Embudo, or Rinconada, library patrons, and/or attend programs or events on library property or that are sponsored by the library. 
What you will do.  Complete a survey or give feedback in other ways that are relevant to you.
Time to complete the survey or interview.  About 8 minutes.
How will the findings be used?  The purpose of this survey is to gather community input about this idea.
Privacy and Confidentiality  Your involvement in the research is voluntary, and you may choose not to participate. You can refuse to answer any of the questions at any time. There are no names or identifying information associated with your responses. If published, results will be presented in summary form only, no identifying information will be published.  There are no known risks in this research. Data will be summarized to make decisions about making changes to library grounds.  All identifiable information (e.g., your name) will be removed from the data collected in this project.
