WA Mental Health Awards 2026 - Nomination Form

The Prevention or Promotion Award recognises an outstanding contribution to the prevention or promotion of mental health in WA for people with moderate, severe or persistent mental health challenges.

The successful nominee can demonstrate how they have increased community awareness and understanding, and reduced the stigma, of moderate, severe and persistent mental health challenges. This includes strong engagement with mental health consumers, the sector and/or the wider WA community.
Submitting a nomination for the Prevention or Promotion Award

Nominations for this award are open to any individual, organisation, group or project. Self-nominations are accepted for this category.

Please address the following questions in a separate document and upload as an attachment to this form:
  • Please demonstrate how the individual, organisation, group or project has helped to promote mental wellbeing in WA and/or prevented the impact of mental health challenges.
  • How does the individual, organisation, group or project engage with people with lived experience of mental health challenges and/or the wider community?
  • Please provide details of the impact, achievement or positive changes that can be seen as a result of this individual, organisation, group or project.
The written response to the above questions should not exceed 2,500 words, excluding supporting documents or images.
