The Colorado State Innovation Model (SIM), a federally funded, governor’s office initiative, has launched a service to help primary care practice sites extract electronic clinical quality measures (eCQMs) once and report to several approved entities.
 
The State Innovation Model (SIM) wants to invest in practices who are interested in learning how to submit electronic clinical quality measures (eCQM) for use within Medicaid, Medicare or any other payer’s advance payment model.  The submission of electronic clinical quality measures can help clinical care teams understand their clients and conditions and teach those clinical care teams how to understand and make better use of the health data in their electronic medical records. eCQM measures will be the following:
Screening for Depression and Follow-Up Plan,
Controlling High Blood Pressure
Diabetes A1C
Body Mass Index (BMI)
Maternal Depression
Adolescent Obesity
Substance Use Disorder (SUD), Tobacco
Developmental Screen
Asthma

Below is information about each organization in the program and guidelines for making your choice of organization. You will be asked to select a first and second choice. Choosing a partner to participate in this program will not affect your support from SIM practice facilitators or clinical health information technology advisors. The eCQM organizations have formed a collaboration called Health Data Colorado (HDCo). The HDCo organizations you may select from are:

Colorado Community Managed Care Network (CCMCN):
CCMCN is a not for profit organization supporting Colorado healthcare communities by providing data management, quality reporting, data analysis and performance improvement strategies empowering organizations that provide services to Colorado's vulnerable residents. CCMCN builds upon its 20 years as nationally recognized HRSA Health Center Controlled Network (HCCN) for Colorado and with this experience, CCMCN can ensure that providers have the healthcare technology strategies they need to be successful.  

CORHIO:
CORHIO is an established health information exchange with 13,000 network users primarily along the Front Range, Eastern Plains and some of the mountain towns. CORHIO is a contractor with the Colorado State Innovation Model working with practices to extract electronic clinical quality measures automatically from electronic health record systems using CORHIO’s established processes and years of experience in this area.

Quality Health Network (QHN): 
Quality Health Network (QHN) is a not for profit health information exchange partnership that has a mission to facilitate the availability of information to optimize the health of our communities, improve economic efficiencies of patient care and bring value to our stakeholders.

Some general principles to consider when choosing your HDCo organization include:

1.      Consider the long-term relationships and partnerships you already have or want to develop in the future.
2.      If you are currently working with an organization providing you with network or health data services for another project, you should consider choosing that organization to maximize alignment and coordination across projects. Consider the geographic reach of the organizations.
3.      If you wish to change HDCo organizations, your existing organization would like the opportunity to resolve the relationship first and then you may choose another HDCo o
