Survey Purpose and Privacy Act Statement

Please review the following information prior to taking the survey.
This survey is created and administered in accordance with AFI 33-332, The Privacy Act of 1974, AFI90-302, and DAFMAN36-2664.

Authority: 1. Secretary of the Air Force,  2. Chief of Staff of the Air Force / Chief of Space Operations, 3. MAJCOM/USSF/DRU/FOA Commander

Purpose: The purpose of this survey is to evaluate an organization, function, or process as part of a routine Unit Effectiveness Inspection (UEI). The responses of this survey will go directly to IG inspectors as a part of forming risk based sampling strategies for upcoming on-site inspections. The survey is to be administered no later than 90 days prior to the in-person capstone event (180 days for ARC).
AFI 90-302, para 3.4.6: UEI survey. During each UEI cycle, MAJCOM/FLDCOM IGs will administer a survey to the inspected wing/delta to capture candid, confidential beliefs, attitudes, and opinions about matters relevant to the four MGAs (T-1). The purpose of the survey is to gather observations since the last on-site evaluation, assist in determining inspection team composition, and inform the RBSS for Capstone on-site evaluations.
Confidentiality: In accordance with AFI 33-332 and DAFMAN36-2664 Para 5.2, survey participants are ensured FULL CONFIDENTIALITY except as required by law. All responses will be kept exclusively in IG circles. IGs will not provide Wing personnel (to include Wing Commanders) copies of survey results under any circumstances.

Disclosure: Adverse or administrative action may NOT be taken against individuals because of their response(s) to an official DAF survey, except as required by law.

Disclosure Exceptions:

1)      If survey responses indicate an immediate health or welfare concern to the individual or others, MAJCOM IG personnel will make every attempt to ascertain the identity of the individual.

2)      If respondent provides comments about criminal behavior, the MAJCOM IG must ensure that comments about criminal behavior are provided to appropriate law enforcement personnel.

3)      If survey responses include allegations of sexual assault, MAJCOM IGs must comply with reporting requirements in accordance with DAFI 90-6001.
