Informed Consent
The title of this research study is “Rural Healthcare Managers Use of Job Embeddedness Factors to Retain Registered Nurses.”

I am Shirleta (Shirley) Soden, a doctoral student under the guidance of Dr. Cynthia DeMarco in the College of Doctoral Studies at Grand Canyon University. The purpose of this qualitative descriptive research is to explore how rural healthcare managers in the southern United States describe the ways they use the concepts of job embeddedness as strategies to retain RNs to support workforce and organizational stability.

KEY INFORMATION

How do I know if I can be in this study?
o   I am a member of one of the following Facebook or LinkedIn groups: (1) Tennessee Nurses Association, (2) American Association of Managed Care Nurses, (3) Kentucky Nurses Association, (4) Arkansas Nurses Association, (5) Future of Nursing West Virginia, (6) Rural Health Clinics Information Exchange, (7) Rural Nurse Organization, (8) Oklahoma Nurses Association, (9) Philippine Nurses Association, (10) Kentucky Rural Health Association, and or resides in the southern United States ( Alabama, Arkansas, Florida, Georgia, Kentucky, Louisiana, Maryland, Mississippi, Missouri, North Carolina, Oklahoma, South Carolina, Tennessee, Texas, Virginia, Delaware, West Virginia, and District of Columbia)
o   I am working more than 12 months as a director of nursing, chief nurse officer or nurse executive, chief executive officer, clinical nurse manager, clinic nurse manager, or other healthcare manager role
o   I can describe how job embeddedness (why RNs stay in the organization or current positions) was used as an approach to implement RN retention strategies

What I am being asked to do? If you agree to be in this study, you will be asked to:
o   I will complete two questionnaires online (online demographic questionnaire and Job Embeddedness Questionnaire®). The questionnaires are 2-5 minutes in length.
o   I will be scheduled for a one-on-one interview. The one-on-one interview will be about 45-60 minutes long and will be virtual at my choice date and time.
o   I will check the interview transcripts for accuracy. The interview transcripts will be sent via email for me to make any changes or edit at my own discretion.
o   I will be asked to refer a study participant/s who can be included in the study during the one-on-one interview or when I complete the online demographic questionnaire.
o   I am willing to answer personal, identifiable, demographic questions related to gender, age, networking site are you associated with, your job title (5-10min).

When:  The research was approved by the Grand Canyon University (GCU) IRB.

Where: Interviews will be through the Zoom videoconferencing application. Questionnaires will be sent online using a SurveyMonkey® link.

How: One-on-one interviews will be virtual. Questionnaires will be sent to all participants electronically.

Audio (Video) taping: I will be recording and videotaping your responses during the interview. All information obtained in this study is confidential. The recordings will be stored in a locked cabinet in my home office. The recordings will be kept for three (3) years after the end of the study. The manual data will be shredded, and the electronic data will be erased.

Any possible risks or discomforts? There are no known risks with this study.

Any direct benefits for me? No.

Any paid compensation for my time? You will not get paid for taking part in this study.

How will my information and/or identity be protected? I will make sure that your info is safe. I will keep the data on a password-protected, personal computer, USB drive, Microsoft SkyDrive®, and external hard drive that will be kept in a locked cabinet at my home. I will use codes and leave out any personal info to keep your privacy throughout our meeting and in the result chapters of this dissertation. I will keep your info for 3 years. After 3 years I will erase the computer data and destroy paper data.
PRESENTATION OF INFORMATION COLLECTED
The results of the study may be used in reports, presentations, and publications. No personal information will be link to the study participants, instead pseudonym will be used when discussing specific participant data.
PRIVACY AND DATA SECURITY
Will researchers ever be able to link my data/responses back to me? Yes.
Will my data include information that can identify me (names, addresses, etc.)? Yes.
Will researchers assign my data/responses a research ID code to use instead of my name? Yes, a code will be used in place of names.
If yes, will researchers create a list to link names with their research ID codes? Yes, I will be assigning a specific code for you.
If yes, how will researchers secure the link of names and research ID codes? How long will the link be kept? Who has access?
o   I will keep the data on a password-protected, personal computer, USB drive, and external hard drive that will be kept in a locked cabinet at my home.
o   I will use codes and leave out any personal info to keep your privacy throughout our meeting and in the result chapters of this dissertation.
o   After three years I will erase the data on the computer and destroy paper data.
o   I will be the only person with access to this information.

How will my data be protected (electronic and hardcopy)? Where? How long? Who will have access? Approximate destroy date?

FUTURE RESEARCH
Names and email addresses were removed from the data (identifiable private information) collected for this study. The interview transcripts and the responses from the questionnaires could be used for future research studies or distributed to other investigators for future research studies. This can be done without additional informed consent from you.
