Informed Consent to Participate in the study
“Regis College Community Members’ Perceptions of an On-Campus Greenhouse and Community Garden”

Introduction
Please read this form carefully. You are being asked to participate in a research study to find out the perceptions of the Regis College community regarding an on-campus greenhouse and community garden. You were selected to participate in this study because you are a student, faculty or staff member at Regis. You are not eligible to participate if you are not 18 years of age or older. You are not eligible to participate if you are not a student or employee at Regis. Please ask any questions you may have before you agree to participate in the study.

Purpose of the Study
The purpose of this study is to find out how students, faculty and staff feel about an on-campus greenhouse and community garden. The study also seeks to find out whether you and others on campus would be interested in gardening activities. In addition, the researchers would like to know your ideas regarding the benefits and challenges of a greenhouse and community garden on the Regis campus.

Description of Study Details
If you agree to participate in this study, we will ask you to complete a short survey. The survey will ask about your ideas regarding an on-campus greenhouse and community garden. It will take 5-10 minutes to complete. There are 16 questions on the survey.

Benefits of Being in this Study
There are no immediate personal benefits of being in this study. The results will provide information from students, faculty and staff about their interest in an on-campus greenhouse and community garden. You might benefit in the future by working with others in a community garden on campus.

Risks and Discomforts of Being in this Study
There are no risks associated with being part of this study beyond what is experienced in daily life while working on a computer.

To the extent the study requires or involves physical interaction with other people or otherwise occurs within space shared with other individuals there is a risk of transmission of and/or infection by communicable disease including, but not limited to, the 2019 Novel Coronavirus (COVID-19). The study will be conducted in compliance with local, state, and federal guidance related to COVID-19, but despite these efforts the risks of transmission and/or infection cannot be completely eliminated.

Payments
There is no payment for participating in this study.

Cost
There is no cost to you for being in this research study.
Choosing to participate in the Study and Choosing to Quit the Study
It is your choice to participate in this study. If you choose not to be part of this study, it will not affect your grades. If you choose not to be a part of this study, it will not affect your employment. You are free to not answer questions or quit at any time, for any reason. There is no penalty for not taking part in the study or for deciding not to complete the survey.

Getting Dismissed from the Study
The researcher may dismiss you from the study at any time for the following reasons:
  • You are not 18 years of age or older
  • You are not currently a student, faculty, or staff member at Regis College
Privacy
The records of this study will be kept private. This study is anonymous. Your name will not be connected to the survey. The responses are meant to be combined with other participants’ answers. They are not meant to gather information about specific individuals. All electronic information will be secured using a password-protected file and available only to the researchers and statisticians involved in the study. No published reports will include any information that will make it possible to identify you.
