MARCH 2026 CHANCE TO PURCHASE ENTRY FORM INFORMATION

Please read the information below prior to completing the March 2026 Chance to Purchase Entry Form.

• Winners should be notified by email no later than March 20, 2026.

• Be sure to dedicate enough time to complete the entire entry form at one time.

• A progress bar can assist participants in completing the entry form. The bar must read 100% at the end of the entry form to be sure choices are selected and fields have information entered, before clicking “Done.”

• Each participant will be required to choose a liquor store location where they will be purchasing a bottle if chosen as a winner. This will be done through a dropdown list of liquor stores.

• Incomplete forms will not be considered.

• Multiple entry forms submitted for the same participant will disqualify all entries.

• It is the responsibility of the participant to select and enter the appropriate information exactly as requested because errors may disqualify the participant.
Other Information:

NO CHANGES (ADDITIONS OR DELETIONS) CAN BE MADE TO AN ENTRY ONCE THE PARTICIPANT SUBMITS THE FORM.

• If a cell phone number (or landline) was entered during the entry form process as the contact number to be used by the liquor store to contact a winner, please enter the liquor store phone number in the phone contact list. This should assist in not missing this important call as spam and potentially forfeiting the chance to purchase a winning bottle.

• Participants can enter an email address to receive a link to their completed entry form responses. See information below.

On the "SUBMIT YOUR ENTRY" screen and prior to clicking "DONE", click on the box in front of the statement "Send me a copy of my responses via email". A text field will appear, and an email address must be entered by the participant to receive an email.

An email should be sent from SurveyMonkey with a link showing the participant's responses. Click the button with the words “View answers”. If an email does not appear in the participant's inbox, check the junk/spam folder. The email may need to be moved from junk/spam into the inbox to read the email.

Confirmation emails are not sent to participants who partially complete an entry form or do not meet minimum qualifications (not an Oregon resident or under 21 years of age).

SurveyMonkey only sends one confirmation email to a recipient within a 5-minute period. A best practice is to wait a minimum of 10 minutes before entering a different individual’s Chance to Purchase information when using a single (family) email address to be sure SurveyMonkey can send an additional email link. Otherwise, completing and requesting confirmation emails within 5 minutes will only generate one email confirmation for the first entry.

If a respondent enters an email address requesting a copy of their responses but then clicks the “Prev” button to return to an earlier page in the form, their email is erased, and the participant will need to re-enter their email address at the end of the form again to trigger an email confirmation.

NOTE: Since the email notification regarding entry responses is being generated from a third party, OLCC has no control over these email notifications and is unable to assist with any issues.

During a previous event, a few participants did not receive an email link to their responses. Please consider taking screenshots of the entry form as the information is entered for a record if needed.
