Ione and Wilton Survey on Maternal & Early Childhood Health 

PURPOSE OF THE SURVEY
The purpose of the survey is to gather information from Ione Band of Miwok Indians (Ione) and Wilton Rancheria (Wilton) members to understand the community needs and resources for a home visiting program that will provide social support and services to promote maternal and early childhood well-being in a culturally-appropriate environment. Ione Band and Wilton Rancheria have partnered with Acorns to Oak Trees, a non-profit tribal organization, to provide these valuable services to our communities. The information you provide will be used only for the purpose stated above. You may choose to provide contact information at the end of the survey for follow-ups and updates about the program. Your name and contact information will NOT appear in the results.

WHO MAY TAKE THE SURVEY
Ione and Wilton members and their families, including those who do not have children aged 0-5 years, may take the survey
