THIS QUESTIONNAIRE IS FOR PRISONS AND YOUNG OFFENDER INSTITUTIONS ONLY. IT SHOULD BE COMPLETED BY ONE PERSON FOR EACH PRISON OR YOI.

There are separate evaluation forms to be completed by public libraries, learning providers and workplaces running the Six Book Challenge 2012.

Please look through the whole form before starting to complete it as you will need a range of statistics to hand. It may be easier for you to print it off to complete the statistics section before you start filling in the form online. However you'll be pleased to know that it is shorter than in previous years.

You can save your work on the form by clicking the appropriate button at the bottom of each page and then return to it later.

Please be sure to complete this questionnaire by FRIDAY 6 JULY 2012 at the latest so that we include your feedback in our analysis, even if you are still running this year's Challenge. The detail you provide will inform our development of the Challenge. It will also help us decide which organisations are eligible for our gold, silver and bronze awards for completion rates.

All completed forms received by FRIDAY 6 JULY 2012 will be put into a prize draw for new books received from publishers through our Reading Partners scheme.
