Virtual Exhibitor Questionnaire

In order to best serve our healthcare community, the 2021 Rocky Mountain Stroke Conference will be offered for FREE and virtually in six individual 1-hour lunchtime sessions.  Webinars will be presented on Thursdays from noon-1:00pm (MST), April 22-May 27, 2021. This virtual conference series is not to be missed! So far we have Physicians, Nurse Practitioners, Physician Assistants, Nurses, EMS, Pharmacists, Students, Therapists, Stroke Coordinators, and other Health Professionals, waiting to register and attend. 

If you are interested in exhibiting on the website during this innovative event, please complete the following questionnaire by Friday, March 19, 2021.  Once completed, watch for a confirmation email with an invoice and W-9.
