Thank you for attending the 2021 Annual Symposium as a Virtual meeting. Your responses are critical to future planning of our educational activities. In accordance with ACCME regulations, we are required to conduct a post-activity survey in order to gauge the effectiveness of our educational interventions.
This form will also serve as your request for CME credit for the portions of the meeting you attended through the virtual meeting dates. You will indicate the sessions you attended during the virtual dates and attest to those hours attended and we will email you a cme certificate. NOTE - CME certificates will not be generated automatically. They will be sent via email 1-2 weeks following the close of the Symposium.
If there were sessions you missed, you can return to the platform when the on-demand recordings are available and view and take the quiz to obtain cme credit. This form is for the virtual meeting dates only.
