Dr. Wisdom F. Coleman Memorial Scholarship

The 516 Foundation, Inc. through the generosity of Delta Dental of Tennessee and the Smile 180 Foundation is giving away a one-time, $1,000 scholarship to four participants who attend the 2024 Determined to be a Doctor Someday (D.D.S.) Virtual Symposium in honor of Dr. Wisdom F. Coleman. The winners will be recognized at the conclusion of the event slated to occur Saturday, November 9, 2024 at the virtual symposium. *The winners must be present during the announcement to win.*
The application process is simple. Complete this one-page application, and in 250-words or less, describe why you want to pursue a career in the healthcare profession.

CRITERIA: Be 14-18 years of age and attend the 2024 Determined to be a Doctor Someday (D.D.S.) Symposium.

DEADLINE FOR SUBMISSIONS: Thursday, October 31, 2024 at 5:00 PM CST

NOTIFICATION: The winners will be notified at the 2024 Symposium on Saturday, November 9, 2024.

PLEASE NOTE: Funds will be given directly to the undergraduate institution that the selected students choose to attend. Proof of enrollment will be required before funds are disbursed. Any questions regarding billing or how the award is distributed should be directed to the college or university. We have no control over how accounts are handled. If enrollment in an undergraduate institution is not completed five years after the scholarship is awarded, the recipient forfeits receipt of it.
