NYSILC is a statewide organization that develops a plan for centers for independent living to serve people with disabilities in New York State. We invite you to participate in this year’s consumer needs survey. Your answers will help us make sure that your needs are included in the statewide plan for services. Those services are delivered by the local centers for independent living to people with all types of disabilities. Our goal is to make sure that all people with disabilities can fully be part of their communities and receive services they need. Your opinions matter! 
    
At the end of the survey, you may enter to win one of five $50 gift cards from Amazon. Please complete the survey by June 25th, 2022 to be entered into the drawing.  The results of this survey will be posted on the NYSILC website in the fall www.nysilc.org.
