Award of Excellence Requirements and Eligibility

Chapter Guidelines for Awards of Excellence:
Recognizing CFPC members who have made outstanding contributions in the past 12 to 24 months to the discipline of family medicine.

REQUIREMENTS:
Awards of Excellence recognize CFPC members who, in the past 12 to 24 months, have made outstanding contributions to one or more of the following:
- patient care
-community service
-hospital or health care institutions
-College activities (National and Chapter)
-teaching, research, or other elements of the discipline of family medicine

 ELIGIBILITY:
-Nominee must be a member in good standing
-Nominations can be from members or the public.
-Groups and family medicine residents are eligible.
-Runners-up to Family Physician of the Year can be considered.
-Members can be re-nominated in subsequent years for new outstanding achievements.
-Chapters may recognize their recipients at their ASA and other provincial events.
-CFPC will acknowledge recipients at FMF and publish the names in the CFP, and the awards website
-Up to 50 Awards of Excellence are given per year with a maximum of two from Manitoba

APPLICATIONS CAN BE COMPLETED IN STAGES BUT MUST BE COMPLETE BY NOVEMBER 30, 2019 TO BE ELIGIBLE FOR SUBMISSION TO THE AWARDS COMMITTEE.
