2018 Senior of the Year Nomination Form

The City of Thousand Oaks recognizes and celebrates senior adult volunteer contributions through the Senior of the Year Awards. If you have a volunteer that has provided you amazing service, please consider nominating them by filling out this form. 

Any organization or individual can nominate a senior for these awards.

Nominations must be received by April 1, 2018, 5:00 p.m.

Selected nominees will be interviewed in April 2018. The dinner and awards ceremony will be held on June 7, 2018 at the Goebel Adult Community Center.
