Unsheltered/Service-Based Survey for KY Balance of State Continuum of Care Point-in-Time Count
Welcome to the 2018 K-Count Unsheltered and Service-Based Survey Data Entry Page. 

You have entered the K-Count survey for UNSHELTERED INDIVIDUALS AND HOUSEHOLDS (OR SURVEYS CONDUCTED IN SERVICE-BASED SETTINGS WHERE PEOPLE MAY HAVE BEEN UNSHELTERED OR STAYING IN EMERGENCY SHELTER)

If you are entering data collected from a paper copy of the survey and used the Sheltered Survey form (refer to the top of the paper survey form you are entering to check) please use the Sheltered Survey Data Entry Page.

Before you begin entering responses from a paper survey form, please take a moment to review the information recorded to ensure questions have been answered. Most importantly, you must make sure you have the age range (Q25), gender (see Q16 and Q27), race (see Q17 and Q27) and ethnicity (see Q18 and Q27) for each member of the household included on the survey. The total number of persons in the household should be recorded on Q24. 

If any of this demographic information is missing for a member of the household, you will not be able to submit this form. If missing data exists for these categories, we suggest you contact the person who conducted the survey and ask them if they recall the information. If the respondent refused to give a response, the surveyor should have recorded his/her (the surveyor's) best guess.
