Please read the call for proposals handout before submitting your proposal. The submission deadline is Friday, December 8 at 11:00 am CT. It is suggested to draft your proposal in a separate Word document before submitting. Changes cannot be saved and returned to. Please note that once you have submitted your proposal, you will be taken to a confirmation page.

Proposals that are incomplete or are submitted after the deadline will not be considered for selection. If you have any difficulties or questions, please contact TeamTraining@aha.org. 

* 1. Contact information for the submitting poster author. Please note that if accepted, other poster authors can be added to conference materials.

* 2. Please submit a brief biography (up to 250 words). Please do not submit resumes or CVs.

* 3. Indicate the themes (up to two) to which your poster relates.
