2017  Volunteer Registration & Release

Thank you for your interest in volunteering with the Down Syndrome Association of Middle Tennessee (DSAMT). Each volunteer must register and sign (electronically or in person) the volunteer waiver. For volunteers under age 18, we require a parent or legal guardian to complete and sign the form.
