Your contact information and action plans are required by the Indiana Association for Career & Technical Education (IACTE) for data collection and reporting purposes, and for the purposes of providing the validation necessary for substitute reimbursement for your attendance at the workshop. You must complete the survey in that time frame in order to receive your PGP/PDP and sub pay reimbursement form, which will be sent to you via the email address you provide, within a few days after you submit this survey. This survey will be closed on October 5 at 11:59 PM.
