Why am I being asked to complete this survey?
The Center for Native American Youth (CNAY) is surveying Native young people across the country to find out what resources you need to address important issues affecting your community. You are being asked to complete this survey because you are a Native youth between the ages of 18 and 24 years old.

What questions will I be asked in the survey?
The survey will ask you questions about yourself, and the community where you live. The survey will also ask you about resources you may need to address issues you face in your community. Finally, the survey will ask you about some of the services that are currently available in your community.

The survey may include some questions that upset you. You do not have to answer any question that you do not want to answer. If you get upset or would like to talk to someone about how you feel, please email cnayinfo@aspeninst.org or call (202) 736-2905. You can also visit www.cnay.org for a list of resources that may be helpful to you.  

You do not have to complete the survey if you do not want to. It is up to you. You can start the survey now and change your mind later. No one will be upset with you if you change your mind.

How long will it take me to complete the survey?
The survey should take you about 15 or 20 minutes to complete. 

How will the information I share in this survey be used?
Information from the survey will be used to help the CNAY understand the needs of Native youth and the resources that are currently available to Native youth in their community and state. CNAY publishes a report that includes all of the information that is gathered in this survey. Your name and individual responses to any survey questions will not be identified in the report.

Your participation in this survey also gives you the option to stay connected with CNAY for future opportunities to share your story and perspective as a Native youth. We will share what we learn from this survey so you can use the information to help educate others, fundraise for projects, and advocate for youth in your community.
Who can see my answers to the survey questions?
Only the people working on this project will know your individual answers, and they will not see your name attached to the answers you share. Any information or data we have about you will be stored on a secure and protected computer.

Everything you share in Part 1 of this survey will remain confidential and will not be associated at all with your identifying information. This part of the survey asks question about resources you or other youth in your community need to address challenges you may be facing.

Part 2 of this survey is not confidential. Part 2 asks for your name, contact information, and what issues you’re most passionate about. Part 2 is a fully separate survey and none of the identifiable information you provide will be associated with any responses in Part 1.

Please fill out Part 2 if you would like to:

* be eligible for a chance to share your story through a CNAY event or other opportunity; and/or

*be eligible to receive a prize for completing this survey. (Prizes may include CNAY swag, N7 athletic gear, and other fun items.) Note that your name will be entered only ONCE into the drawing to win a prize. Please do not take the survey more than once.
