Thank you for your interest in the Delta Dental grant, which allows students in Kindergarten to grade 4 the opportunity to visit the Duke Energy Children's Museum which includes the Inside the Grin exhibit.

Before you begin completing this grant application please note:

-This grant is only available for Kindergarten to grade 4 classes in Hamilton County, Ohio who have a financial need. Financial need will be determined by the percentage of students in your school on Free or Reduced lunch.

-One classroom per application. Several classrooms from one school may be awarded the grant to ensure all students in a grade level have the same opportunity but each classroom teacher must have a separate application.

-This grant provides admission to the Duke Energy Children's Museum and bussing for students and chaperones between December 1, 2018 and March 31, 2019. Schools within the 275 loop will book buses through ArtBus. Schools outside the 275 loop will receive a maximum of $250 reimbursement for bussing.

- Contact information for the classroom teacher must be provided. Resource coordinators, PTO and other support staff may complete the application but all communication will be directly with the classroom teacher that brings the students on the visit. This ensures all information about reservations, expectations and feedback will reach the correct person.

-Teachers who receive the grant will be responsible for contacting Cincinnati Museum Center to book admission to the museum AND ArtBus (or another provider) to book a bus.

-Teachers are expected to complete and turn in an evaluation from Delta Dental at the time of their visit and an online survey from Cincinnati Museum Center after their visit.

-Students are expected to participate in a self-paced learning opportunity that guides and enhances their understanding of oral health.
