Within this survey you will be asked about your recent GP visit where you presented with symptoms of a urinary tract infection (UTI) also commonly referred to as cystitis.
This survey should take approximately 10 minutes to complete. Your answers will be used to improve the treatment of UTIs in the future.
There are no right or wrong answers as your honest opinions are what matters. All information collected will be treated in the strictest confidence and it will not be possible to identify individuals or their responses from this survey. The responses to individual patients will not be given to the GP.
