Purpose and Procedure. You are being invited to participate in a 20-minute health survey. This survey will help us learn more about promotional methods that can benefit individuals in Lebanon who experience organ failure. This survey is part of a research project that is being conducted by Georges E. Abou Khalil, MPH for his Doctorate program in Communication at the State University of New York at Buffalo.

Voluntary Nature: Your participation is completely voluntary and you may withdraw your consent to participate at any time during the process. If you choose to do so, any information derived from your participation will be deleted from the evaluations and findings.
Confidentiality. This survey is anonymous. No one will be able to identify you, nor will anyone be able to determine which company you work for. No one will know whether you participated in this survey.
Risks and Benefits. Risks associated with this study are minimal. The greatest risk in this study is a potential breach of confidentiality; however, the chances of this are very low. The information you provide will help us understand how best to satisfy the health needs of Lebanese citizens. The information collected may not benefit you directly, but this study should provide long term health benefits to those who experience organ failure in Lebanon.
