Question Title

* 1. This survey is intended to be completed by a parent that has participated in a family nature club with their child(ren). There are five questions and it should take less than five minutes to complete the survey. The primary purpose is to get your feedback on the 20 effects of participating in a family nature club that I have identified through my doctoral research (www.nurturednature.org).

I know that your time is very valuable - the information you share is also very valuable! Completion of this survey by March 15th will enter your family into a raffle for a $50 gift certificate to REI, an outdoor recreation store.

Your participation in this survey is COMPLETELY VOLUNTARY. Your responses are confidential and your name and contact information are not required. If you have any questions about this survey, please contact me at cdamore@prescott.edu
