Questionnaire for veterinary surgeons and veterinary nurses involved in the treatment of dogs and cats with diabetes

Short-term aim:
To gain information on the perceived benefits and shortcomings of insulin treatment in veterinary practices.

Long-term aim:
To use this information to develop superior diabetic treatment and improve the quality of life of diabetic pets and their owners.

Incentives:
1. Knowing that you are actively helping to advance veterinary diabetic care.
2. United Kingdom vets and nurses only: Submitting one veterinary surgeon and veterinary nurse questionnaire (completed by a Veterinary Surgeon or Nurse) AND one or more owner questionnaires (completed by a client of your practice with a diabetic dog or cat which is currently receiving treatment with insulin injections) will automatically mean the submitting veterinary surgeon/nurse will have the chance to win free registration for a day of Diabetes CPD.

Instructions:
Filling in the questionnaire should not take longer than 5-10 minutes of your time and 10 minutes of your client’s time. Please note that only one VET questionnaire should be submitted per practice/ practice branch; however, multiple owner questionnaires can be submitted. Please pay close attention to words that specify the statement or question (i.e. ……according to the owner, ….. in the cat, …… in the dog). Please, feel free to provide additional comments at the end of the questionnaire.
