Make a Difference award nomination form

If there’s a person, group or organisation you think deserves recognition for the volunteering they do in the borough, why not nominate them for a Make a Difference award?

The deadline for nominations is 5pm, Tuesday 3 May 2011.

Winners will be chosen by a judging panel and invites for the ceremony will be sent out to winners and runners up shortly after judging takes place.

The Make a Difference Awards 2011 ceremony will take place on 7 June 2011.

Please note: If you do not fill in every section of the nomination form your nomination will not count.
