Neighbourhood Forums attendees - new contacts

As a new Neighbourhood Forum attendee, Coventry City Council asks you to provide contact details so that we may send you notes of Neighbourhood Forum meetings and invitations to future meetings. We would also be grateful if you could complete some quick survey questions to give us your views on Neighbourhood Forums.

You will be asked to provide some personal information; this is so that we can monitor which groups of people attend Neighbourhood Forums.

Please be advised that the information you provide will not be shared outside Coventry City Council but may be accessed by teams within the organisation. All information you provide will be held securely.

If completing a paper copy of this form at a Neighbourhood Forum, please write clearly and in capitals and place your completed form in the deposit box or hand to the Support Officer or Senior Support Officer.

If you have completed this survey before (either online or at a previous meeting) your information has already been recorded on our contact database - you need only complete the meeting attendance sheet.
