CAP - Food Survey

* 1. The purpose of this survey is to find out where you are getting your food. This voluntary survey will ask you to describe the different places you and/or your family usually purchase or receive what you eat. The survey will make your contribution anonymous. No names or contact information is asked for. The survey will not collect your computer IP address. The Leadership Windsor Essex research team will be compiling a report with this information. This report will not be sold or passed on to businesses.
