  • Please complete this form in its entirety to submit your Discovery Trunk request. Partially completed forms cannot be saved.


  • Allow five business days to process your request. You will receive confirmation materials via email. Note that your loan reservation is not confirmed until you receive these materials.


  • On the final page of this form, you will have an opportunity to submit questions regarding your loan request.
