Welcome to the Columbus Department of Public Health - Event Support Request Form
We would like to thank you for your commitment to making Columbus Georgia a Healthier place to Live, Work and Play.
We are committed to assist you in your health promotion efforts and have created this form to get some basic information about your community event so we may better serve your needs.
In order for us to coordinate staff and resources to assist you with your event, we request the following:
* Health fair participation requests must be submitted no later than 45 days prior to the event.
* Requests for Health Educators to present on a health topic must be submitted no later than 30 days prior to engagement.
All requests will be reviewed and a response will be given within 10 business days from the completion of this form.
