The Center for Health Statistics (CHS) uses this survey to capture information about individuals requesting access to Oregon vital statistics data. You must complete a separate application for each project.
Once you have answered the following questions, print and sign the signature form at the end of this survey. Signatures are required from you and your manager. Email, fax, or bring the signed form to the Center for Health Statistics. Contact information is provided at the end of this survey.
Note there is no way to save your work and return to complete the survey at a later time.
