1. Spring Break at UF and Santa Fe this year is March 5th-9th

Please answer this survey in full. If you do not answer each question, your survey cannot be successfully submitted. For each question, please provide the best answer you can give, even if you are not 100% sure of your plans. "I don't know my plans yet" is not a good answer. Once you've submitted your answers and moved on to the next page, you will not be able to go back and edit your previous answers.

This survey is simply a questionnaire designed to give us an idea of how our production levels will change during Spring Break. Answering the questions on this survey DOES NOT count as "asking off." Any requests for days off or to move hours around during Spring Break must be made ON TIME on your My Schedule page on COSA. Any late requests will be denied.

As a reminder, On-Time Requests need to be submitted by 5:00pm on the Monday of the week preceding the week you are requesting to make changes to.  So in order to have your request for Spring Break guaranteed to be approved, make sure to have your request in by 5:00pm on February 26th.  Requests submitted after then will be denied.

Please DO NOT take this survey more than once. If your plans change and/or you want to change your answers after you're done, send an e-mail to GNVOFFICE@COPYTALK.COM with the subject "Spring Break".

When you finish the survey, it will take you to a "Thank you" screen and you will have to click "Done". If you don't answer each question, it will not take you to the Thank You screen and you won't be able to submit your survey.
