Informed Consent

You are invited to participate in an adult community survey of health perspectives and experiences. This study is being completed by Tobacco-Free Broome and Tioga, a non-profit organization funded by the NYS Department of Health and one that has served Broome and Tioga Counties for over 20 years. Your input will help us improve health services in our communities. The survey focuses on community members’ health perspectives and experiences as individuals living or working in the region. This very short 3-5 minute confidential survey in Broome and Tioga Counties addresses many important issues related to public health, specifically including several questions about tobacco and nicotine product use and policies. We are not selling anything or trying to solicit donations.

We request your informed consent to be a voluntary participant, and you may discontinue participation at any time and can do so without any prior notice or explanation. Your responses are anonymous and will not be linked to your identity in any way.

For answers to pertinent questions about the research objectives and research subjects' rights, or about the research sampling methodology or statistical analyses employed please contact the Director at TFBT.
