D October 2017 Re: Utah Community Action’s Housing Case Management Assessment & Application Process In an effort to improve our accessibility to the community, Utah Community Action has shifted our Housing Case Management services assessment and application process to a daily over-the-phone or walk-in process. Potential clients can see if they are eligible for our Housing Case Management assistance by calling (801) 359-2444 or stopping by our location at 764 South 200 West, Salt Lake City. Both over-the-phone and walk-in assessments will occur: Monday-Friday, 8:30 am to 5:00 pm.
