Dear Participant,

Purpose of Research
This study is focused on gathering information from people who have experienced a vision-related disability and are between the ages of 18 and 64 years of age. I hope to gather
information about the ways people with vision-related disabilities cope/manage with their disability and how these methods of coping/managing may relate to people's ability to perform basic daily activities, sense of overall satisfaction with their life, and interaction with the broader community.

Specific Procedures to be Used
Included in the questionnaire used for this study are demographic and background questions, questions about how you tend to cope with your vision-related disability, and questions related to your daily functioning. The questionnaire is in an online format. At the bottom of this page is a button that when clicked will direct you to the questionnaire. If you choose to participate, please click the button below and complete the questionnaire. At the end of the survey, you will be asked to submit your responses. You will also be provided with a button that if clicked will direct you to a separate website designed to collect the emails of participants who would like to be entered into a drawing. If you would like to be entered into the drawing, you can click this button and submit your email address.

Duration of Participation
The completion of these forms will take approximately 20-30 minutes.

Benefits to the Individual
You will not experience any obvious benefits from participating in this study; however, you may find that answering the questions gives you a new awareness of how your vision-related disability has impacted you.

Risks to the Individual
The risks involved in participation are no more than what would be involved in everyday life.

Compensation
After participation, you may choose to be entered into a drawing. You will be directed to a separate website following completion of the survey, and you will be asked to give your e-mail address. The e-mail of one participant will be drawn. The person that has his or her e-mail drawn will receive a $25 Audible.com gift certificate. Submitting your e-mail will indicate that you participated in the survey, but the researchers will not be able to link your e-mail with your survey responses. There will be no more than 1 in 100 odds for the drawing.

Confidentiality
The privacy of your responses will be protected through multiple methods. You will not be asked to give your name or any identifying material other than background information. Information connected to the IP address of the computer you used to complete the survey will not be collected. While complete privacy cannot be guaranteed when completing a form on the internet, every effort will be made to keep your information confidential. There will be no greater risk than otherwise exists when using the internet. Participant responses will be evaluated and presented collectively, and the data will be kept indefinitely. The data will be used for the purposes listed above, and only the researchers can access the data. However, departments at Purdue University responsible for research oversight may review the research records.

Voluntary Nature of Participation
You do not have to agree to participate in this research project, and if you agree to participate, you can withdraw your participation at any time.
