Service Learning Liability Waiver

This form should be submitted by Loyola students over 18 who are participating in any service learning activity which will require them to be off-campus.

If you are over 18 and have already filled out a Placement Acceptance Form, you've already completed this. You don't need to fill out this form.

If you are under 18, you cannot fill out this form. You and a parent or guardian must complete a paper waiver. Get one here.
